Introduction

When you sprain an ankle, it swells, reddens and hurts. That's acute inflammation: a fast, intense, localized response with — most importantly — an expiration date. The immune system shows up, repairs and withdraws. Without that mechanism we wouldn't survive a cut or a trivial infection.

Chronic low-grade inflammation is the exact opposite in everything but the name. It doesn't hurt, it isn't visible, it isn't localized and it doesn't end. It's a persistent immune activation, low in intensity, spread throughout the body and sustained for years. You don't notice it because there's no symptom to give it away; you detect it, if at all, in a blood marker or when the disease it had been quietly preparing finally appears.

This kind of inflammation has been linked to the major chronic diseases of our time: cardiovascular, metabolic, neurodegenerative and autoimmune. It isn't the sole cause of any of them, but it's the common ground on which many germinate. That's why it has gone from a footnote in physiology textbooks to a central concept in the conversation about longevity.

The good news is that its main drivers aren't genetic or random. They're behavioral, everyday and modifiable. This article explains what lights it, how to know whether you have it and which four levers put it out.

1. Distinguish the two inflammations. The acute kind is intense, useful and brief; the chronic kind is mild, useless and permanent. This distinction matters because it leads to opposite conclusions: you don't want to eliminate inflammation — that would leave you defenseless — you want it to switch off when its work is done. The goal isn't to "de-inflame": it's to recover the ability to resolve.

2. Ask for a high-sensitivity CRP. High-sensitivity C-reactive protein (hs-CRP) is the most accessible marker. Unlike conventional CRP, which detects obvious acute inflammation, the high-sensitivity version captures low, sustained levels. A persistently elevated value, in the absence of infection, is a signal worth attention.

3. Reduce visceral fat: it's an inflammatory organ. The adipose tissue surrounding your organs isn't a passive warehouse. It continuously produces and releases pro-inflammatory molecules. Losing visceral fat is probably the intervention with the greatest effect on systemic inflammation, and it responds well to both a moderate calorie deficit and strength training.

4. Sleep: a lack of it switches the system on. A single night of insufficient sleep raises inflammatory markers the next day. Sustained over time, sleep debt maintains a low-grade activation that no anti-inflammatory food compensates for. If you had to choose a single lever, this one competes head to head with diet.

5. Manage chronic stress, not one-off stress. Cortisol is, paradoxically, anti-inflammatory. The problem is that sustained exposure produces cortisol resistance: cells stop responding to it the same way, and the switch-off signal stops arriving. That's why chronic stress inflames while a one-off scare doesn't. The distinction is, once again, duration.

6. Eat fiber: you're feeding what de-inflames you. Gut bacteria ferment fiber and produce short-chain fatty acids, above all butyrate, which nourishes the cells of the colon and has local and systemic anti-inflammatory effects. Fiber doesn't act directly: it acts through what it feeds. Legumes, vegetables, oats, whole fruit.

7. Prioritize omega-3 over industrial omega-6. This isn't about eliminating omega-6, but about correcting a ratio that the Western diet has skewed through mass consumption of refined vegetable oils and ultra-processed foods. Oily fish, walnuts and flax seeds supply the side that's usually missing. The ratio matters more than the absolute value of either.

8. Quit tobacco before anything else. No anti-inflammatory strategy compensates for smoking. Tobacco smoke maintains continuous immune activation in the lungs and blood vessels. If you smoke, this point isn't one more on the list: it is the list.

9. Take care of your mouth: inflammation starts there more often than you'd think. Periodontal disease is a chronic inflammatory focus with direct access to the bloodstream, and it's consistently associated with elevated systemic markers and cardiovascular risk. Brushing, flossing and regular cleanings are anti-inflammatory measures, even though nobody presents them that way.

10. Move, but respect recovery. Exercise produces a transient acute inflammation followed by a net anti-inflammatory adaptation. That's the effect you're after. Excessive training without enough rest turns the acute into the chronic and cancels the benefit. More isn't better: the right dose with enough recovery is.

11. Be skeptical of "anti-inflammatory" foods as the solution. Turmeric, ginger and green tea contain compounds with demonstrated anti-inflammatory activity in the lab, but the real doses and bioavailability in a normal diet fall far short of compensating for poor sleep, inactivity or visceral fat. Use them as a complement to a pattern, not as a substitute for one.

12. Think in patterns, not in individual foods. No food inflames or de-inflames in isolation. What inflames is a sustained pattern: excess ultra-processed food, little fiber, little movement, little sleep. What de-inflames is the opposite pattern, maintained for months. Hunting for the guilty food or the miracle food is the most effective way to change nothing.

Why the system doesn't switch off

Acute inflammation has an active resolution mechanism: it doesn't just run out, it's actively shut down by specific signals. In chronic inflammation that resolution mechanism works poorly or is permanently overwhelmed by stimuli that never stop. Understanding this changes the approach: the problem isn't only too much "on" signal, but that the "off" signal doesn't arrive or isn't heard.

The gut connection

A permeable intestinal barrier lets bacterial fragments pass into the bloodstream, where the immune system recognizes them as a threat and responds by activating continuously. It's one of the proposed mechanisms for why the state of the microbiota and intestinal lining relates to systemic inflammation.

Inflammation and aging

The term inflammaging describes the progressive rise in low-grade inflammation with age. Its magnitude isn't inevitable: older people who are active, well-rested and of healthy body composition show notably better inflammatory profiles than their sedentary peers. Age sets the trend, habits set the slope.

A final thought

Chronic low-grade inflammation is awkward precisely because it causes no discomfort. There's no pain forcing you to act and no symptom taking you to a doctor. There are only years of silent wear and, at the end, a diagnosis that seems sudden and isn't. The levers that control it are the same as always — sleep, move, eat real food, manage stress — and that apparent banality is exactly why we ignore them. You don't need a sophisticated protocol. You need to sustain the basics for long enough.
